Membership Application / Horse Nomination

Submit your membership/horse nomination through the online forms below, or download a PDF (2023–24 Syracuse PHA Membership Form), print, and mail.

PLEASE NOTE: there are TWO separate Forms REQUIRED. You MUST submit the Membership Form* and then come back to this page for the Horse Nomination Form**.

Follow these three easy steps:

1. Pay the appropriate fee via PayPal or Venmo.

2. Fill out the Membership form.

3. Fill out the Horse Nomination form(s), with a separate submission for each horse.

Note: MEMBERSHIP AND NOMINATION FEES MUST BE RECEIVED BY THE PHA SECRETARY BEFORE ANY POINTS WILL BE COUNTED. NOMINATIONS RECEIVED BY JANUARY 1ST WILL EARN POINTS FROM DECEMBER 1ST.
